Repatriation letter for a deceased Congolese
Requirements
- A typed or handwritten signed letter addressed to the Ambassador of Congo in Sweden, requesting the establishment of a Congolese death certificate. Your correspondence must include your full name, address, email, and telephone number
- A certified copy of the original birth certificate of the deceased person
- Proof of Congolese citizenship such as a Congolese passport, Congolese national identity card, or a certificate of Congolese citizenship for the deceased
- A copy of the medical file from the hospital that certifies the death of the person and its cause
- An official letter from the Funeral Home confirming their responsibility for the administrative procedures for the repatriation of the body
- A certificate from the Funeral Home certifying that the body has been embalmed according to the sanitary conditions established by the Swedish, Norwegian, Finish and Icelandic government and that the body poses no health threat to its environment
- A legible copy of the license of the Funeral Home responsible for the body
- An official identification document of the family representative of the deceased
- A flight reservation or copy of a flight itinerary confirming the intention to repatriate the body to the Republic of Congo
- A prepaid return envelope from a wwedish postal service, such as PostNord, DHL, or other available services, is required to return your documents.
